Social Work

The Society for the Promotion of Hospice Care

Position: Social worker

Contract period: two years (non-renewable)

Main responsibilities, in hospice care/terminal care and grief support services

1. Provide professional clinical social work services, including case evaluation, care planning, intervention counseling, crisis management, and service evaluation, to provide emotional support, counseling and treatment services to terminal patients, family caregivers and bereaved individuals.

2. Plan, implement and lead a grief support group, grief counseling activities, life education workshops, public education lectures, and daytime activities for terminally ill patients and their caregivers.

3. Assist institutions in publishing, research, knowledge exchange and professional training, and promote hospice care and grief support knowledge to the public, volunteers and medical and nursing teams.

4. Responsible for volunteer recruitment, training, matching, supervision and management, building and optimizing the volunteer support system and community service network.

5. Closely coordinate with medical teams, social welfare institutions, community groups and cooperative units, and coordinate the implementation of inpatient and outpatient services.

6. Provide interdisciplinary case conferences, resource referrals, and follow-up services

7. Plan project application for funding and budget management, write professional reports, case records, service data and progress reports to ensure that services meet the organization's and Social Welfare Department's professional standards.

8. Participate in service planning, quality control, service review, and continuous improvement work.

Entry Requirements

· Hold a social work degree and be registered as a social worker with the Hong Kong Social Worker Registration Board (either Level 1 or Level 2 is acceptable)

· At least 3 years of work experience, interested in grief and end-of-life care. Welcome those who have received relevant training and have experience, mature and stable personality, emotional management, and good at getting along and cooperating with others.

· If the applicant has no experience in this service, the Association will provide on-the-job training and guidance (including knowledge of end-of-life care, grief counseling skills, and bereavement service procedures)

· Applicants are required to have a good work attitude, enthusiasm, communication and listening skills, and the ability to build interpersonal relationships. They should have empathy and patience with bereaved families, and the ability to respond and adapt.

· Proficient in Chinese and English oral and written communication, fluent in Mandarin is preferred

· Familiar with MS Office office software (Word, Excel, PowerPoint), with basic data processing and report writing abilities

Company Overview
The Haven Society (SPHC), a non-profit organization based in Hong Kong, was founded on July 1, 1986 by Gabriel O'Mahoney, Dr. Hsieh Chien-Chuan, Dr. Yu Rong-Guang, Pastor Li Ding-Hsing, and John Russell, among others. Since its inception, the Society has been dedicated to promoting high-quality hospice and palliative care services in Hong Kong. In addition to providing hospital care, home care, and day services, the Society also covers bereavement counseling, professional training, and public education activities, with the aim of educating the public to face death and end-of-life care, and to convey the important message of cherishing life. The Haven Society established Hong Kong's first independent hospice care center, the White Memorial Hospice Center, in 1992, and formally established the Tan Yee Shih and Du Pei-Chen Home for the Dying in 1997. In 2017, the Society launched the Jockey Club Haven Home with great fanfare, with the support of the Hong Kong Special Administrative Region Government and the Jockey Club Charities Trust. SPHC, a non-profit organization based in Hong Kong, has been dedicated since its establishment on July 1, 1986 by Gabriel O'Mahoney, Dr. Shek Kin Chuen, Dr. Yu Wing Kwong, Rev. Li Dingxin, and John Russell among others, to promoting the development of high-quality palliative care services in Hong Kong. Beyond hospital services, home care, and day services, SPHC also encompasses bereavement counseling, professional training, and public education activities aimed at educating the public about death and end-of-life care, conveying messages of valuing life. In 1992, SPHC established Hong Kong's first independent palliative service center, the White Paper Palliative Care Center. It officially launched the Tanya Tsui Duk Ping Chan Home Service Center in 1997, and in 2017, it introduced the SPHC House with significant support from the Government of the Hong Kong Special Administrative Region and the Hong Kong Jockey Club Charities Trust.
